Yes, you can have DID or OSDD if you know about your alters. Both DID and OSDD develop in childhood, meaning they are present even though many people are unaware they have a system until later in life, sometimes much later. Some people hear their alters or even converse with them without understanding that they are alters.

Many people mistakenly believe that DID always involves complete unawareness of alters. In reality, awareness exists on a spectrum and can fluctuate over time. Some people are aware of only a few alters while others may know many. Some systems communicate internally while others have very little communication. Some parts may intentionally remain hidden or minimize awareness of the system in order to protect it.

It is also common for people to recognize alters long before they understand what they are experiencing. They may think of alters as different sides of themselves, moods, internal companions, or conflicting versions of themselves. Becoming aware of alters does not eliminate dissociative barriers or mean that a person is “making it up.” Often, awareness of alters is simply the beginning of understanding the system more clearly.
