Hypervigilance and Fatigue: Why Threat Monitoring Uses So Much Energy
(Summary) Hypervigilance can require far more energy than you realize. When your nervous system is constantly monitoring your surroundings, other people, internal experiences, and possible signs of danger, it is performing ongoing mental work—even when that work happens outside your conscious awareness. For people with DID, OSDD, or complex trauma, this constant threat monitoring can contribute to exhaustion and leave fewer mental resources available for concentration, memory, decision-making, emotional regulation, and everyday tasks.
Sometimes my clients wonder why they are so exhausted. They haven’t run a marathon. They may even have had the day off. What’s going on? The short answer is that DID and OSDD involve a great deal of invisible mental work. For instance, you’re so used to having a conversation while simultaneously watching every person in the room, listening for unusual sounds, and constantly asking yourself whether you’re safe that you no longer consciously realize how exhausting it is.
Your nervous system may never be truly “off duty”
If your nervous system was trained to be hypervigilant by a traumatic environment in childhood, it doesn’t turn off simply because you consciously know you are safe. It’s an ongoing process, even if it is in the background.
Threat monitoring requires energy
Hypervigilance is an active process. Your nervous system is constantly scanning for possible dangers. Your nervous system may be assessing your environment, the people around you, your internal experiences, and comparing them to past experiences of danger. While you may not always notice this work happening, it does use your resources. Less energy remains for everything else
The more mental resources your nervous system uses to monitor for threats, the fewer remain for concentration, memory, decision making, regulating emotions, and other tasks. Hypervigilance doesn’t just increase stress; it also quietly reduces your capacity to perform other tasks. You may not connect lunch with a friend at a busy restaurant with why you’re struggling to decide what to make for dinner or why it’s harder to concentrate later that evening.

Why am I exhausted even when I haven’t done anything?
Physical activity is only one source of energy demand. If your nervous system spends much of the day monitoring your environment, other people, sounds, internal experiences, and possible threats, you may have done considerable mental work despite accomplishing relatively little physically.
Can hypervigilance affect concentration and memory?
Yes. When more mental resources are being used for threat monitoring, fewer may remain available for concentration, memory, decision-making, emotional regulation, and other cognitive tasks.
Why does hypervigilance continue when I know I’m safe?
A nervous system shaped by a traumatic environment can continue monitoring for danger even when you consciously recognize that your current environment is safe. Conscious knowledge of safety does not necessarily cause learned threat-monitoring patterns to immediately stop.
Can social situations make hypervigilance more exhausting?
They can. Being around other people may give a hypervigilant nervous system more information to monitor, including facial expressions, voices, movements, changes in behavior, and the surrounding environment. As a result, an ordinary social activity may require more mental resources than you realize.
